Submission of More than Four Diagnoses on Form CMS-1500
The Centers for Medicare & Medicaid Services (CMS) recently sent correspondence to all Medicare contractors providing clarification on the submission of more than four diagnosis codes on the CMS-1500 claim form.
CMS instructs that contractors support only four diagnoses in Item 21 of the CMS-1500 claim form. Providers needing to submit more than four diagnosis codes must submit an electronic claim; not a paper claim.
WPS Medicare recommends that providers submitting claims on the CMS-1500 claim form maintain all diagnoses in their medical records and be prepared to supply them if requested.
