CMS-855B and CMS-855I Medicare Enrollment Forms - Revised

The Centers for Medicare & Medicaid Services (CMS) posted revised versions of the CMS-855B and CMS-855I Medicare enrollment forms to its Website on September 1, 2009. The revised forms indicate "CMS-855B (02/08) (EF 07/09)" and "CMS-855I (02/08) (EF 07/09)" in the lower left corner of the forms. CMS has advised contractors to accept the previous versions of these forms through November 30, 2009, and to require the current, revised versions of these forms for applications received on and after December 1, 2009.
