Iowa and Indiana: National Modifier and Condition Code To Be Used to Identify Disaster-Related Claims

During the last national disaster, a modifier and condition code were established in Change Request (CR) 4106. They were developed to facilitate claims processing and track services and items provided to victims in disaster areas. Modifier CR (Catastrophe/Disaster Related) and condition code DR (Disaster Related) were established for providers to use on disaster related claims. The modifier and/or condition code can be used by providers submitting claims for beneficiaries who are victims of the flood-stricken States of Iowa, effective for dates of service on and after May 25, 2008, and Indiana, effective with dates of service on and after June 6, 2008.

For physicians or suppliers billing their local carrier or Durable Medical Equipment Medicare Administrative Contractor (DME MAC), only the modifier (CR) may be reported and not the condition code. A condition code is used in fiscal intermediary (FI)/Part A and Part B Medicare Administrative Contractor (A/B MAC) billing. For institutional billing, either the modifier or condition code may be reported. The condition code would identify claims that are or may be impacted by specific payer policies related to a national or regional disaster, while the modifier would indicate a specific Part B service that may be impacted by policy related to the disaster.
