Medicare Physician Fee Schedules
How to use different types of files:
The fee schedules have been placed in two different file types. There is a pdf version for easy viewing/printing, while in the other column, the files have been placed in a zip file. Within the zip file, the fees have been placed in an Excel and pdf format for use.
MENTAL HEALTH FEES NOTE: The mental health procedure codes listed in the full 2008 Medicare Physician Fee Schedule are effective for dates of service 01/01/08 - 06/30/08 only. For mental health fees for dates of service 07/01/08 - 12/31/08, please see the Mental Health section below.

Updates to the 2008 Medicare Physician Fee Schedule
Review of Allowances for Carrier Priced Codes
In conjunction with the J5 system conversion, the Carrier Medical Directors have completed a review of the carrier-priced fees (C - Status on the MPFSDB) that previous contractors established for the states of Iowa, Missouri, Kansas, and Nebraska. There were several fee changes implemented. The fee schedules that follow reflect the revised pricing amounts. The pricing if effective based on the claim process date. Because of the data merge requirements for Eastern and Western MO, pricing changes were made in Missouri for 2007, 2008, and 2009. For Iowa, Kansas and Nebraska, only 2009 pricing was impacted.
In addition to the changes in pricing, some codes that may have been previously allowed in some of the J5 states will now be denied or subject to review. Those codes include the following:
