Missing/Insufficient Documentation for Injections

Documentation for injections must reflect the drug, dosage, frequency, method, and site of administration. Providers need be cognizant that dollars paid in error, whether small or large, can adversely impact the paid claims error rate. We recommend that providers consider performing periodic self audits if they provide these services.
