All correspondence is coordinated through our Home Office location in Omaha, NE unless the provider is specifically working with an associate in one of our Field Offices. In this case, the provider can refer to our Field Office contact listing and address links for more information.

Please do not send any individually-identifiable information through the internet/E-mail. This includes, but not limited to, specific beneficiary information, such as Health Insurance Claim (HIC) number, Social Security Number (SSN), specific claim information, IRIS data and Bad Debt listings. If you wish to speak with someone about a specific claim, we encourage you to contact our Claims Department. If you wish to speak to someone regarding IRIS or Bad Debt information, we encourage you to contact our Audit & Reimbursement Department.
